We are giving COVID-19 vaccines 1st, 2nd and 3rd dose Moderna to all patients 18+ and Pfizer-BioNTech to 5 Years of Age and older. The booster shot at 6 months or more after their initial series who received a Moderna or Pfizer COVID-19 vaccine.
Please bring your drivers license. If you have insurance through a Medicare Advantage plan we will need a copy of your Medicare card.
